The XML service returned error: 'connection-refused'

We recently performed an upgrade in our Farm, from 7.6 to 7.15 LTSR CU3.

Randomly we observed about 3-4% of failed sessions. (SesiisonPreparation, ConnectionTimeout)

In storefront servers event viewer this is the error: "The XML service returned error: 'connection-refused'. (event id 28)"

 

VMs seems to refuse connections randomly, always with the error Citrix Desktop Service 1050 ("Connection validation failed on domain " for user " for reason 'Deny') followed up by error tdica 21 (ICA Service - Event ID 21 - Broker Agent failed to validate connection (E_EXCHANGE_FAILED) disconnect remote console connection immediately. )

These errors seem to indicate the VMs as engaged in some other activities, even if they're registered and services all running.

Users access VMs through Netscaler Gateway, the problem happens both at logon and when trying to reconnect to the desktop.

 

All Citrix Components are leveled at 7.15 LTSR CU3, storefront machines are load balanced GSLB, but no modifications have been made to SF or Netscaler settings.

Anyone out there has some solutions?

Already viewed a lot of CTX. I cannot get traces cause problem is random...
