Netscaler reboot w/ message "OCR started due to FW fault detected!"

My Secondary MPX-22040 rebooted this morning inexplicably and I found the following lines in my messages file:

 

Oct 12 04:26:25 <kern.crit> kernel: mrsas0: OCR started due to FW fault detected!

Oct 12 04:26:25 <kern.crit>  kernel: mrsas0: Reset failed, killing adapter.

Oct 12 04:26:25 <local0.warn> 10.135.112.33 10/12/2016:08:26:25 GMT  0-PPE-1 : CLUSTERD Message 76027 0 :  "Timer called after 1552 ticks "

Oct 12 04:26:25 <local0.alert> 10.135.112.33 10/12/2016:08:26:25 GMT  0-PPE-1 : PITBOSS1 Message 76028 0 :  "Wed Oct 12 04:26:25 2016 Pitboss hit snooze alarm - system experiencing resource distress : snooze 6 . Rebooting system. Collect and review tech support bundle."

 

Anyone seen this before?

i know it is late , but maybe some that has the same issues might see this.

 

the problem  is with the Raid Controller firmware.

 

 kernel: mrsas0: Reset failed, killing adapter.
kernel: mrsas0: OCR started due to FW fault detected!

 

Go to shell and do this:

   # MegaCli64 -AdpAllInfo -aALL | grep "FW Package Build"

     if you get this : FW Package Build: 23.12.0-0011  you need to upgrade the RAID firmware. Citrix can provide you the firmware file.

 

Afer you upgrade and reboot you go back to shell and check again:

   # MegaCli64 -AdpAllInfo -aALL | grep "FW Package Build"

         you should have this version:    FW Package Build: 23.33.0-0023

 

if you still have issues after the upgrade then probably the device needs to be replaced.
